The Hardest Thing About Fertility No One Talks About

One of the most challenging and often unspoken parts of a fertility journey is this: you can do everything “right” and still not have the full picture.

Many couples are told that their results are normal. Hormones appear balanced, ovulation is confirmed, and sperm parameters fall within standard ranges. On paper, everything looks reassuring. And yet, despite doing everything they’ve been advised, pregnancy doesn’t happen.

This is where fertility can become deeply frustrating and confusing. The word “normal” can feel comforting at first, but in reality, it often just means that nothing obvious has been identified. It doesn’t necessarily mean that everything is functioning optimally, or that all contributing factors have been fully explored.

Fertility is rarely straightforward. It’s influenced by multiple systems in the body working together, and when one or more of those systems is slightly out of balance, it may not always show up on standard tests.

Beneath the surface, there can still be important pieces of the puzzle that haven’t yet been considered. For example, sperm DNA fragmentation can affect embryo quality even when basic sperm parameters look normal. Subtle hormone signalling issues may impact how effectively the body communicates throughout the cycle. Inflammation and immune imbalance can influence implantation, while metabolic health plays a role in hormone regulation and egg quality.

Other factors, such as the vaginal or uterine microbiome, are becoming increasingly recognised as important in fertility, yet are not always routinely assessed. Nutrient deficiencies can also quietly affect both egg and sperm quality without being immediately obvious.

Individually, these factors may seem small, but together they can make a meaningful difference in whether conception occurs.

So if you’ve been told that everything looks normal, but something still doesn’t feel right, it’s important to trust that feeling. You’re not imagining it, and you’re not alone in experiencing it.

Often, it simply means that a deeper, more holistic look at your fertility is needed — one that considers how all aspects of your health are interacting, rather than viewing results in isolation.
